In the first quarter of 2026, Alabama Skilled Nursing Facility Operations Holdings, LLC owned Falkville Rehabilitation and Healthcare Center, as reported by the Centers for Medicare and Medicaid Services (CMS). The facility, situated in Morgan County, has been under Alabama Skilled Nursing Facility Operations Holdings, LLC ownership since March 24, 2023.

Falkville Rehabilitation and Healthcare Center operates as a for-profit facility.

For that quarter, the overall CMS rating for the nursing home stood at 1, falling short of the statewide average of 3.0 for Alabama. CMS determines ratings by examining different areas, including health inspection results, staffing adequacy, and the level of care provided to residents.

Data also shows Falkville Rehabilitation and Healthcare Center had an average daily census of 97.8 residents in the first quarter of 2026, out of a total of 116 licensed beds. The nursing home does not operate as a hospital-based facility.

By the end of the same period, the facility had incurred $146,615 in fines across two incidents and received three penalties.

At that time, Morgan County, Alabama, had four for-profit nursing homes in total.

U.S. Census Bureau estimates indicate the number of older adults in the country is rising while younger age group populations decline. Between 2020 and 2024, the count of Americans age 65 and older grew 13%, while the number of individuals under 18 decreased by 1.7%. Over the past century, the nation’s older adult population has surged from 4.9 million in 1920 to over 61 million in 2024.

A 2021 study finds that about 28 million people in this age bracket—almost half—may need long-term care and support, with more than a third projected to require nursing home services in time.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
